Someone didn't follow the checklist and safe the cannon with one of these:

I delivered a lot of bombs to fighter aircraft, our crew chiefs and load toads were very good about safing those cannons and weapons racks..someone in the Belgian Air force doesn't know how to use a checklist or is just plain lazy.

I was on the parking ramp at Incirlik AB, Turkey in 1982 when a maintenance man in a Turkish F-4 fired off a missile about 150 yards in front of me.

AD's are more common in combat zones than most people realize. There were several on DaNang airbase while I was there in '65 & '66. Missiles, napalm, small arms, 20mm, even a fully loaded USAF attack bomber that crashed on take off about 1am about 200yds from me and the 50gal LOX trailer I was towing. Bombs & 20mm started cooking off in the inferno.

And that doesn't count incoming. Dangerous damn place, especially when there was a big push on.

Even a "safe" weapon can malfunction. I once had to put out a trail of fire a 2.75 rocket left through the desert flora.

Middle of the night and absolutely no one around one of the rocket fired from the pod. The tower reported it and we went out to put out the brush. Once that was done they wanted us to "find" the rocket. In the middle of a very dark night, right. We told them to call EOD, they said they'd be out in the morning. They weren't looking for live ordnance in the dark either.

Not accidental-negligent. If a piece of equipment is in safe working order, any problem is due to operator incompetence.
