have you checked the barrel to see if the bolt has come loose?
( The ammo fouled my barrel so badly that my groups opened up to about 12+ inches at 7 yards. )
it will lossen up very little. my wife was having troubles too. then she read this web site and its helped her racking the slide.

I always try to rack the slide on all my semiautos. I know if you do malfunction drills they tell you to tap and rack the slide and its best to do the same thing everytime.
