It's an off ctr strike. Are all you cases like that? Was the cyl bolt locked in the cyl notch when the trigger was pulled?
Cock the gun very slowly and see if the cyl locks before or after the hammer cocks in the full cock notch.
Push the firing pin all the way forward and see if it's bent.

Been somewhat off center for ever. On the left of the picture is the misfire. I dropped the hammer on this primer 4 times but it wouldn't fire. I personally believe it was a bad primer. Close to 5 figures though this sixgun & this is the first.
I'm thinking bad primer as well.

Bad or contaminated primer. A high primer will do the same thing but four hammer drops should have seated nearly anything.
Was this a factory round or a reload? If a factory round, a call or e-mail might get you an entire box free as a replacement and they might want to have you send them the defective round.

Reload. This is the 3rd firing of these cases in my Ruger. This load was the Lee 452-230-TC, 4.0 grs. Promo & a Winchester Large Pistol primer.

DITTO: Bad primer . If it was a reload punch it out and look it over you might be surprised at what you discover.
