Interesting Problem

I've got a new Ruger Guide Gun in 375 Ruger. Rifle shoots great and is a pleasure to shoot. Normally, I single round load at the range but recently started loading 3 into the mag to see how it feeds and ejects.
I noticed that most of the time the first round will feed & fire fine but the brass does not get tossed out when the bolt is withdrawn. It is held fast to the bolt face even when I cycle the bolt briskly. This does NOT happen on the second or third round out of the magazine, both of which toss out quite a distance.
What could be causing this to happen?

Usually when the brass isn't ejected it is the Ejector doesn't line-up with the groove cut in the bottom of the Bolt. But, that wouldn't explain why the #2 and 3 rounds don't have the same issue.

Try this. Put a small amount of red lipstick on the top of the Ejector, then cycle the bolt a couple of times. Check for red along the Bolt groove. If there is some extending the length of the groove you can bend the Ejector slightly till it doesn't leave a red streak.

Step 2. Check that the forward edge of the Ejector doesn't protrude too far into the Magazine well.

Are you dropping the first round in the chamber, then forcing the bolt closed due to the extractor needing to "jump" the rim?
then the next rounds coming from the magazine slide under the extractor.

If so, the first round may have the rim damaged (pushed in) and held tighter to the bolt face than the undamaged rims being feed from the magazine

My thoughts too. Sometimes this makes a difference, so I've heard.

That's what I've heard and found out about semi-autos is that they should strip from the magazine and not closed on a round in the chamber. I guess some bolt rifles may be sensitive to that as well.
