I was digging through my extra Blackhawk manuals this evening and found one of these floating around in the folder. Per Dougan's book they were used from August 55 through September 56 when the Combined instructions and parts list dated September 1, 1956, was released.

Rare paper! These were inserted in the early Single-Six instruction manual that also went as paperwork with the original Blackhawks

One of the first .357 flattops that I bought sight unseen off GunsAmerica years ago came with one of those in the non-catalog box. It is a 4-digit gun and cost me $480 total shipped, but that was in 2002. I still have it, rated at 99% condition. Letters as Aug, 1956. I think I got another gun with one of those many years later but I don't see it noted in my spreadsheet so it may be one I haven't moved off my FFL books yet.

The latest RENE price guide lists that insert sheet at $115.00.

Be careful,, there have been very good "copies" out there.
