LCP Max

New to this Forum, Today was my 3rd session with My LCP Max, Worked great the first two, but today had several issues where the slide didn't lock back after the mag was empty? This occurred with the 10 and 12 round mags Any suggestions

Are you right handed with a left thumb over right thumb grip? Does the thumb ride the release? That may not be noticeable when gripping. The recoil may induce a slide release from your thumb.
