Is it possible to take the grip medallions from one set of grips and install them in another set of grips without destroying them?

If so, a "how to" would be awesome.

Yes, it can be done. CAREFULLY!!!!
First, take the grip panel you are going to remove them from. Place the panel on a thick soft folded towel, medallion down. Then using a proper sized flat nosed punch, push the medallion out of the grip panel. Then, you can install the madallions in a different set with the holes.
I have had some panels where the medallion was glued in, and broken the grip panel too. Be careful.
When I get a set of panels where the wood is already broken or chipped out, I just go ahead and break the wood around the medallion.

A bar of paraffin wax supports the grips. Carve out a hole, slightly larger than the medallion and use light taps with a punch and hammer to remove the medallions.

I got some new grips and they came with silver eagle medallions and I wanted black eagles. Luckily I have some extra factory "cheese graters" with black eagles, so I followed the instructions above and it was a piece of cake. I was really amazed at how easy they came out and went back in. On one set of grips I did use a drop of wood glue as they were a little loose.

I just did this, I had a set of Security Six grips that came off a security trade in Service Six, the grips were so beat up and cracked they were "firewood" status......so I pushed the medallions out with a brass punch, and also took the escutcheons out too. Pretty easy, the medallions are just staked into the plastic round pieces in the grip.
