There are a lot of reasons that a cartridge falls out of favor. Many of the older smokeless military rounds that became very popular hunting/target rounds for civilian use have fallen in popularity.

Looks to be very likely a case of another roll crimped 9mm case. Winchester does this quite often with that round, very dangerous. The bullet and case get into the rifling and locks the case mouth from expansion and bullet release, no place to go but out the case head.

On your grip. Those are stovepipes and are a classic sign of a loosening grip, some call it "limpwristing", I don't like it because people get all defensive. Work on maintaining a good two hand hold and check your reaction to recoil, should be okay after that.
