SRH red dot mount problem...

I just put a new B-Square mount for a red dot on my SRH 44mag. I took two shots and the entire rail was about to fall off. I took the rail off and remounted it, a couple of shots later same result. When I paid attention tot he mounting screws they did not appear to be loose. I took it all apart again and put loc-tite on the screws...I do not know if this would have helped as I stripped out one of the mounting holes.

So here i am...I have to get another rail...
#1 anybody have this trouble with a B-square?
#2 what did I do wrong or what is/was the problem?
#3 It has been recommended to replace it with a Jack Weignd?
#4 other opinions?

please help...I am in a bit of a panic I have my first ever Texas Hog Hunt in two weeks and need to get prepared.

same thing happen to me
shot this red dot 25 yards 2" grope I used jack weignd I don't know if it was the rail the red dot see photo

the next thing im hitting the frame thats holds the target 2' did away with the rail
installed ruger rings and an Ultra DOT have NOT been to the range but I think this combo will work
Ultra dot sets right in

I have the same mounts - put on my SRH .454 and a cheap BSA green-dot. Had fun shooting .45 cowboy type loads loaded into Casull cases - accurate at 25 meters too - but took it off because I want to shoot more full-house .454 loads and I didn't figure it would hold up to it. So now I'm just iron sights. I love pulling it out of my bag at the range when someone in the next lane has a puny .44 mag. :lol:
