I've had a good experience with the Weaver mount, been using it for decades.

Was it a dud or didn't fire at all? If the cylinder gets a little out of alignment they can hang up sometimes also.

Lyman Cast Bullet manual is your best bet.

When using a lead bullet with a crimp groove, the correct OAL is established when the bullet is seated such that the case mouth is located in the crimp groove. If the manual you are using does not show that OAL my first thought is that the manual does not include data for the bullet you are using.

Those throats sound awfully small, how did you measure them? If they really are that small I would expect excess pressure and maybe sticky extraction.

Handgun caliber sets used to be 3 die sets back in the day. Now people have a preference to crimp separately from seating, hence the fourth die.

I recall reading that in order to get the proper orientation of the pin a slot is cut in the flat end of the pin. When the pin is almost flush you use the slot to turn the pin to the correct orientation so that it blends with the polished surface.
